Access Plan Statement

Together with the Federation and Primary Care Networks (PCNs), we’ve developed a robust and adaptable plan to deliver top-quality care for both our Practice and PCN communities. This approach is focused on improving patient access by addressing challenges around appointment scheduling, communication, and the overall patient journey. Highlights of the plan include:

• Offering same-day and next-day appointments—both over the phone and in-person—with a variety of clinicians for urgent and routine care needs.
• Making follow-up appointments available for booking in advance.
• Providing “Triage and Treat” clinics accessible across the PCN.
• An automated telephone switchboard supporting patients in thirteen languages on weekdays.
• Real-time telephone access to clinical teams during weekdays to assist patients promptly.
